We are seeking a Registered Nurse Case Manager to join our team. The ideal candidate will have experience in ICU, Critical Care, Telemetry and Med Surg.

Responsibilities:
– Coordinate patient care with physicians, nurses, and other healthcare professionals
– Develop and implement care plans for patients
– Monitor patient progress and adjust care plans as needed
– Educate patients and their families about their medical conditions and treatment options
– Ensure that patients receive appropriate medical care and services
– Collaborate with insurance companies to ensure that patients receive the necessary coverage for their medical needs

Requirements:
– Active RN license in the state of California
– Minimum of 2 years of nursing experience in ICU, Critical Care, Telemetry and Medical Surgical
– Strong communication skills to effectively communicate with patients, families, and healthcare professionals
– Ability to work independently and as part of a team
– Knowledge of medical terminology and procedures
– Proficient in computer skills
